Registration with the Company.
On registering with Newcastle Futures Limited or Generation North East, we will ask for your consent for the Company to hold the information you provide on a secure database system. The controller of this data will be Newcastle City Council, with Newcastle Futures being the processor of the data.
Any requests for subject access should be made to the Chief Executive, Newcastle Futures Limited, Cloth Market, Cathedral Square, Newcastle upon Tyne or email@example.com.
The consent which is signed to agree your participation with the project is:
1. (Newcastle Futures) working with DWP and Newcastle City Council needs your consent to share relevant personal information about you; this is so we can make referrals to our partners for the purposes of help/services that may be available to you.
This information will be stored securely by (Newcastle Futures) and will only be used for the specific
purposes as detailed below in section 3.
*2. The information (Newcastle Futures) want to share is:
Your name, National Insurance Number and/or date of birth.
Training or any other vocational courses undertaken – type of course, provider and qualifications achieved.
Any barriers preventing you from entering into work.
Specific job goals – Agreed action plans and referrals to job vacancies.
Details of job outcomes – employer, nature of work and start date.
Any appropriate personal information you have provided on this form.
Personal details will be held on a secure central Hanlon database run by and overseen by Newcastle Local
Authority and with your permission, can be shared with other Hanlon users ** (Only organizations registered as Hanlon users)
*3. This information will only be used or shared for the following purposes:
To help our Partners provide you with relevant advice and support in relation to getting back to work.
To help our Partners to encourage you to access the support and advice they provide, or that is available to you.
To help the (Newcastle Futures) adviser to support you by discussing your needs, and to identify
any further help that could be offered by Jobcentre Plus/Partner organisations.
For evaluation and research purposes, if you do agree to take part in this initiative, the information that you give
may be used by the Department for Work and Pensions (DWP) for research into how successful this initiative is and what part of the initiative works well. Any details you give us will only be seen by our team and will only be used for this initiative. When we write the research report no person or household will be identified. We will store your information in a secure place and destroy your records after they are no longer required for our purposes.
To measure whether this initiative is successful or not, we may add what you tell us to details already held by the DWP. We may also pass this information to other Government Departments, but only anonymous information will be given to them so that you cannot be identified. The other Government Department will only be able to use the information for their research purposes and to help the services that they offer.
**Partner organisations may include your local authority, housing association, training and voluntary organisations (this list is not exhaustive).

reasons outlined above only. I also understand that if any information is given to any other organisation for research and monitoring purposes that I will not be identified. I understand that I can withdraw my consent to the sharing of information at any time.
I understand this service is voluntary and give my consent for my personal details to be held on a secure database and used by Newcastle Futures to confirm my identity and be released to employers, funders, partners and referral organisations where it is necessary in the interests of increasing my employment prospects. My personal information may be used for the purpose of management information and external audit requirements by the organisation collecting the data or any organisation with whom the data has been shared for the purposes of increasing my employment prospects. Your personal details will be input onto the Hanlon database by Local Authority employees seconded to Jobcentre Plus for this purpose. My details will be destroyed no later than 6 years from not using the service,unless we are working in partnership with ESF funded projects such as North of Tyne Working Homes which requires us to retain records until the year 2023.
I may request a copy of this consent form. I understand that I can withdraw my consent to the sharing of information at any time and this does not affect my right to claim benefit now or in the future
Due to funding arrangements, we need to collect this data to satisfy the criteria of accessing the funds, if we do not hold this consent from a service user, we may not be able to offer all of the services available.
Registration with futures talk is made through this site on www.newcastlefutures.co.uk/futures-talk revisiting this link allows you to unsubscribe from the service. Newcastle Futures holds the email address with which you subscribe in the relevant distribution list pertaining to the information you would like to receive. We will only use this email address to circulate information on topics requested at registration and will not supply the distribution lists to others.
